My clients is seeking a highly skilled and motivated Advanced Nurse Practitioner to join their dynamic team at a reputable GP surgery. As an Advanced Nurse Practitioner, you will play a vital role in delivering high-quality patient care, managing acute and chronic conditions, and promoting health and well-being within the community. This position offers an excellent opportunity to expand your clinical expertise and make a meaningful impact on the lives of our patients.

Responsibilities:

Provide comprehensive and evidence-based care to patients of all ages, following clinical guidelines and protocols.
Perform initial assessments, diagnose and manage acute and chronic illnesses, and prescribe appropriate treatment plans.
Conduct physical examinations, order and interpret diagnostic tests, and develop care plans in collaboration with the healthcare team.
Independently assess and manage minor injuries and illnesses, providing prompt and effective treatment.
Offer health promotion and preventive care services, including vaccinations, health screenings, and lifestyle counseling.
Provide support and education to patients and their families, ensuring they understand their conditions, treatments, and self-care measures.
Collaborate with other healthcare professionals, such as physicians, nurses, and specialists, to coordinate patient care and optimize outcomes.
Maintain accurate and up-to-date medical records, documenting all patient encounters and interventions in compliance with regulatory standards.
Actively participate in clinical audits, quality improvement initiatives, and professional development activities to enhance your clinical skills and knowledge.
Requirements:

Ideally you will have a MSc degree or equivalent
Prescriber Qualification
Primary Care Experience
Sound knowledge of primary care principles, evidence-based practice, and clinical guidelines.
Proficiency in conducting physical examinations, diagnosing illnesses, and developing appropriate care plans.
Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and multidisciplinary teams.
Excellent organizational skills and the ability to manage a diverse caseload efficiently.
Ability to work autonomously and make informed decisions within your scope of practice.
