Position Overview:

In this role, the successful candidate will play a pivotal role in delivering a diverse range of nursing services, collaborating closely with the multidisciplinary team to ensure the highest standards of care for patients.
Key Responsibilities:
- The Practice Nurse will undertake a range of responsibilities including:
- Conducting thorough health assessments and screenings to identify patient needs.
- Administering vaccinations and immunizations following established protocols.
- Supporting the management of chronic diseases such as diabetes and asthma through the implementation of comprehensive care plans.
- Providing expert wound care management and assisting with minor surgical procedures as required.
- Offering family planning services and providing reproductive health counseling to patients.
- Delivering health promotion initiatives and educational sessions to patients and their families.
- Collaborating with General Practitioners and other healthcare professionals to develop and implement individualized care plans.
- Maintaining accurate and up-to-date patient records and participating in practice audits to ensure quality standards are upheld.
Requirements:
- The ideal candidate will possess the following qualifications and attributes:
- Registered Nurse qualification with current NMC registration.
- Proven experience working as a Practice Nurse within a primary care setting.
- Competency in conducting health assessments and implementing appropriate nursing interventions.
- Proficiency in administering vaccinations, wound care management, and minor surgical assistance.
- Excellent communication skills and the ability to build rapport with patients and colleagues.
- Strong team player with a collaborative approach to healthcare delivery.
- Commitment to ongoing professional development and staying updated on best practices in nursing.
