My client is seeking a highly skilled and motivated Advanced Nurse Practitioner to join their dynamic team at a reputable GP surgery. As an Advanced Nurse Practitioner, you will play a vital role in delivering high-quality patient care, managing acute and chronic conditions, and promoting health and well-being within the community. Responsibilities:
• Provide comprehensive and evidence-based care to patients of all ages, following clinical guidelines and protocols.
• Perform initial assessments, diagnose and manage acute and chronic illnesses, and prescribe appropriate treatment plans.
• Conduct physical examinations, order and interpret diagnostic tests, and develop care plans in collaboration with the healthcare team
• Independently assess and manage minor injuries and illnesses, providing prompt and effective treatment.
• Offer health promotion and preventive care services, including vaccinations, health screenings, and lifestyle counseling.
• Provide support and education to patients and their families, ensuring they understand their conditions, treatments, and self-care measures.
• Collaborate with other healthcare professionals, such as physicians, nurses, and specialists, to coordinate patient care and optimize outcomes.
• Maintain accurate and up-to-date medical records, documenting all patient encounters and interventions in compliance with regulatory standards.
• Actively participate in clinical audits, quality improvement initiatives, and professional development activities to enhance your clinical skills and knowledge.
