A highly respected and long-established medical practice in Crawley is actively seeking a dedicated and compassionate Practice Nurse to join their dynamic and patient-centered team. This is a fantastic opportunity for a registered nurse with a passion for primary care to make a meaningful impact on the health and well-being of the local community.

Key Responsibilities:

  • Patient Care: Deliver a comprehensive range of nursing services, including chronic disease management, immunizations, wound care, and cervical screening. The successful candidate will work closely with patients to provide tailored care that meets their individual needs.
  • Health Assessments: Conduct thorough health assessments, develop and implement individualized care plans, and provide ongoing monitoring and support to patients with long-term conditions such as diabetes, asthma, and hypertension.
  • Team Collaboration: Work collaboratively with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure a holistic approach to patient care. Actively participate in team meetings and contribute to the practice's continuous improvement initiatives.
  • Health Promotion: Lead and participate in health promotion activities, patient education programs, and community outreach efforts aimed at improving public health outcomes. The role includes organizing and delivering clinics on topics such as smoking cessation, weight management, and lifestyle advice.
  • Record Keeping: Maintain meticulous and up-to-date patient records using the practice's electronic health records (EHR) system. Ensure that all documentation meets the required legal and professional standards, and that patient confidentiality is strictly maintained at all times.
  • Protocol Development: Contribute to the development, review, and implementation of practice protocols, policies, and procedures.

  • The Practice Nurse will play a key role in ensuring that the practice meets regulatory requirements and adheres to best practices in patient care.

Essential Requirements:

  • Registered Nurse: Hold a valid NMC registration with evidence of continuous professional development.
  • Experience: Previous experience in a primary care or practice nursing role is essential.
  • The ideal candidate will have strong clinical skills, particularly in the management of chronic diseases, wound care, and health promotion.
  • Communication Skills: Excellent verbal and written communication skills are required, with the ability to engage effectively with patients, colleagues, and external healthcare professionals.
  • Team Player: Demonstrated ability to work both independently and as part of a multidisciplinary team. The successful candidate will be adaptable, proactive, and committed to providing high-quality patient care.

Desirable:

  • Specialist Qualifications: Additional qualifications or experience in areas such as diabetes care, asthma management, or minor injuries would be advantageous and highly valued.
  • IT Proficiency: Familiarity with electronic health records (EHR) systems and basic IT skills are desirable, as the role involves regular use of digital tools for patient management and record-keeping.
