Job Summary
A well-established, patient-centred medical practice in Farnborough is seeking a dedicated and experienced Practice Nurse to join its dynamic healthcare team. This role plays a key part in delivering high-quality care to a diverse patient population.
Key Responsibilities
Provide a full range of nursing services, including wound care, immunisations, and routine health screenings.
Perform patient assessments, create personalised care plans, and support a holistic, patient-centred approach.
Collaborate effectively with GPs, healthcare assistants, and other multidisciplinary team members to ensure seamless continuity of care.
Offer patient education and promote health to empower individuals to manage their well-being.
Maintain accurate and up-to-date patient records, ensuring adherence to all relevant guidelines and protocols.
Qualifications and Experience
Registered Nurse (RGN) with a valid NMC pin.
Previous experience in a primary care or community healthcare setting is highly desirable.
Proficient in administering child and adult immunisations and performing cervical smears.
Experience in chronic disease management is advantageous.
Strong communication skills and a collaborative, team-oriented approach.
Competent in using clinical systems and electronic patient records.
