Job Summary:
A well-regarded, patient-focused medical practice in Liverpool is seeking a dedicated and experienced Practice Nurse to join their healthcare team. The successful candidate will play a key role in delivering high-quality care to a diverse patient base.
Key Responsibilities:
Provide a broad range of nursing services, including chronic disease management, wound care, immunisations, and health assessments.
Perform patient assessments, develop personalised care plans, and ensure patient-centred care is delivered effectively.
Collaborate closely with GPs, healthcare assistants, and other team members to ensure a smooth and coordinated care experience.
Offer health education and advice, supporting patients in managing their own health and well-being.
Keep accurate and up-to-date patient records, adhering to clinical guidelines and protocols.
Qualifications and Experience:
Registered Nurse (RGN) with a valid NMC pin.
Previous experience in a primary care or community healthcare setting is highly desirable.
Proficient in managing chronic conditions such as diabetes, asthma, and COPD.
Strong communication skills and a collaborative approach to team-based care.
Competence in using clinical systems and electronic patient records.
