Job Summary:
A well-regarded and patient-focused medical practice in Lymm is seeking an experienced and committed Practice Nurse to join their healthcare team. The successful candidate will be responsible for providing high-quality care to a diverse patient population.
Key Responsibilities:
Deliver comprehensive nursing services, including chronic disease management, wound care, immunizations, and health screenings.
Perform patient assessments, develop tailored care plans, and ensure the delivery of patient-centred care.
Collaborate effectively with GPs, healthcare assistants, and other team members to ensure smooth and continuous patient care.
Offer patient education and promote health awareness, empowering individuals to make informed decisions about their well-being.
Accurately maintain and update patient records, ensuring compliance with relevant standards and guidelines.
Qualifications and Experience:
Registered Nurse (RGN) with a valid NMC pin.
Previous experience in a primary care or community healthcare setting is highly desirable.
Expertise in chronic disease management, including conditions such as diabetes, asthma, and COPD.
Excellent communication skills and a strong ability to work collaboratively within a multidisciplinary team.
Familiarity with clinical systems and electronic patient records.
