Job Summary:
A reputable, patient-centred medical practice in Morden is seeking a skilled and committed Practice Nurse to become an essential member of their healthcare team. The ideal candidate will contribute to the delivery of exceptional care to a diverse local community.
Key Responsibilities:
• Deliver comprehensive nursing care, including wound management, vaccinations, and routine health screenings.
• Perform patient assessments, develop and implement care plans, and support patient-focused care delivery.
• Work closely with GPs, healthcare assistants, and other team members to ensure seamless care for patients.
• Provide patient education and promote health and wellness, empowering patients to manage their own health.
• Maintain accurate, up-to-date patient records, adhering to all relevant standards and protocols.
Qualifications and Experience:
• Registered Nurse (RGN) with a current NMC pin.
• Previous experience in a primary care or community setting is highly valued.
• Proficient in child and adult immunisations, as well as smear tests.
• Experience with chronic disease management is desirable.
• Strong communication skills and an aptitude for teamwork.
• Competence in using clinical systems and electronic health records.
