Job Summary:
A reputable and patient-centred medical practice in Newton-le-Willows is looking for an experienced and dedicated Practice Nurse to join their dynamic healthcare team. The successful candidate will be integral in delivering high-quality care to a diverse range of patients.
Key Responsibilities:
Provide comprehensive nursing care, including chronic disease management, wound care, immunizations, and health screenings.
Conduct patient assessments, develop personalized care plans, and support the delivery of patient-centred care.
Work collaboratively with GPs, healthcare assistants, and other multidisciplinary team members to ensure continuity of care.
Educate patients on health promotion and lifestyle choices, empowering them to manage their own health.
Maintain accurate and up-to-date patient records in line with all relevant guidelines and protocols.
Qualifications and Experience:
Registered Nurse (RGN) with a valid NMC pin.
Prior experience in a primary care or community health setting is highly desirable.
Skilled in chronic disease management, including conditions such as diabetes, asthma, and COPD.
Strong communication skills and the ability to work effectively within a multidisciplinary team.
Proficiency in using clinical systems and electronic patient records.
