Overview:
A friendly and forward-thinking GP practice in Norwich is seeking a skilled and compassionate Practice Nurse to join their committed clinical team. The ideal candidate will have strong experience in chronic disease management, with particular expertise in diabetes care.

This is a well-established, patient-centred practice that prides itself on delivering high-quality, holistic care to its local community. The multidisciplinary team includes GPs, ANPs, HCAs, and dedicated admin support. The practice is supportive of ongoing development and fosters a collaborative, inclusive culture.
Key Responsibilities:

Lead on diabetes management, including reviews, patient education, and treatment plans

Manage other long-term conditions such as asthma and COPD

Provide cervical screening and routine immunisations

Undertake wound care, health checks, and health promotion activities

Maintain accurate and up-to-date clinical records

Work closely with the wider team to deliver coordinated patient care
Essential Requirements:

Current NMC registration as a Registered Nurse

Proven experience in a primary care or community setting

Strong skills and up-to-date knowledge in diabetes management

Proficiency in general treatment room duties

Excellent clinical, communication, and time-management skills
Desirable:

Qualifications or additional training in diabetes care

Experience using EMIS Web (or similar clinical systems)
