Job Title: Practice Nurse
Location: Salisbury, Wiltshire
Employment Type: Full-time/Part-time
Salary: Competitive, dependent on experience

Job Summary:
A well-established and friendly GP practice in Salisbury is seeking a dedicated and compassionate Practice Nurse to join their dynamic healthcare team. The successful candidate will play a crucial role in delivering high-quality care to a diverse patient population, providing a range of clinical services in a supportive and professional environment.

Key Responsibilities:

  • Conducting patient assessments, including chronic disease management, wound care, immunizations, and family planning services.
  • Performing routine health checks, such as blood pressure monitoring, ECGs, and health screenings.
  • Administering vaccinations and injections, including travel immunizations and childhood vaccines.
  • Supporting patients with long-term conditions, providing advice and management for conditions such as diabetes, asthma, and hypertension.
  • Assisting in the development and implementation of individualized care plans.
  • Offering lifestyle advice to patients, including smoking cessation, weight management, and healthy living.
  • Collaborating with GPs and other healthcare professionals to ensure integrated and coordinated patient care.
  • Maintaining accurate and up-to-date patient records in line with practice policies and procedures.
  • Participating in clinical audits and contributing to the continuous improvement of services.

Qualifications and Experience:

  • Registered Nurse (RGN) with current NMC registration.
  • Previous experience working in a primary care or general practice setting is desirable.
  • Proficient in chronic disease management, wound care, and immunizations.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
  • A commitment to delivering high standards of patient care and promoting health and well-being.
  • Ability to work independently as well as part of a multidisciplinary team.
  • Familiarity with electronic patient records and IT systems used in healthcare.
