Position Overview:

Key Responsibilities:
- Conducting patient assessments and delivering nursing care following best practices and clinical guidelines.
- Assisting with chronic disease management, including diabetes, hypertension, and asthma.
- Administering vaccinations and immunizations as per schedule and guidelines.
- Performing wound care, dressings, and minor surgical procedures.
- Providing health education and support to patients on various topics, including lifestyle modifications, medication management, and preventive healthcare.

- Maintaining accurate patient records and documentation.
Requirements:
- Registered Nurse qualification with valid NMC registration.
- Proven experience working as a Practice Nurse in a primary care setting.
- Strong clinical skills and knowledge of nursing best practices.
- Excellent communication and interpersonal skills.
