Job Summary:
A reputable and patient-focused medical practice in Warrington is looking for an experienced and committed Practice Nurse to join their dedicated healthcare team. The successful candidate will play a vital role in delivering high-quality care to a diverse range of patients.
Key Responsibilities:
Provide a full range of nursing services, including chronic disease management, wound care, immunisations, and health screenings.
Carry out patient assessments, create personalised care plans, and ensure patient-centred care is provided.
Work collaboratively with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure seamless care delivery.
Provide patient education and promote health and well-being, empowering individuals to manage their own health.
Keep patient records up to date and accurate, complying with all relevant guidelines and protocols.
Qualifications and Experience:
Registered Nurse (RGN) with a valid NMC pin.
Experience in primary care or community healthcare is highly desirable.
Expertise in managing chronic conditions such as diabetes, asthma, and COPD.
Excellent communication skills and the ability to work effectively as part of a team.
Skilled in using clinical systems and electronic patient records.
