Key Responsibilities:

Providing autonomous, high-quality care including assessment, diagnosis, treatment, and management of patients with undifferentiated, undiagnosed conditions.

Managing acute presentations, minor illness, and long-term conditions within the scope of advanced practice.

Prescribing medications safely and effectively, in line with professional and legislative requirements.

Ordering and interpreting diagnostic tests including bloods, x-rays, ECGs, and other investigations.

Delivering proactive health promotion and disease prevention advice.

Supporting the achievement of QOF, Enhanced Services, and other national and local health targets.

Working collaboratively with the multidisciplinary team to provide seamless, patient-focused care.

Participating in audits, service improvement initiatives, and maintaining accurate clinical records.
The Ideal Candidate:
-
A qualified Advanced Nurse Practitioner with MSc level training or equivalent qualifications.

Independent Prescriber (V300) with current NMC registration.

Proven experience working as an ANP within Primary Care, urgent care, or a similar setting.

Confident in autonomous decision-making, assessment, and management of complex clinical presentations.

Competent in chronic disease management, minor illness management, and health promotion.

A strong communicator with excellent interpersonal skills and a collaborative, team-oriented approach.
