Salary: £31,000 per annum + £3,500 on-call allowance

Care Coordinator - Oldham / Rochdale / Middleton / Bury
An excellent opportunity has arisen for 4 experienced Care Coordinators to join a highly regarded and growing complex care, domiciliary care and home care provider covering Oldham, Rochdale, Middleton and Bury.
We are looking for experienced Care Coordinators, Domiciliary Care Coordinators, Care Schedulers and Care Planners who are confident managing rotas, coordinating care visits and supporting frontline care staff.
We are also keen to hear from experienced domiciliary care professionals who are looking to move into complex care. Full training and upskilling will be provided.
About the Care Coordinator Role
This is an office-based Care Coordinator / Care Scheduler position focused on the effective coordination of care services and management of Health Care Assistants and care staff.
You will be responsible for:

Coordinating complex care, domiciliary care and home care packages.

Scheduling and coordinating care visits.

Creating and managing staff rotas.
-
Ensuring care calls and shifts are fully staffed.

Managing changes to rotas and responding to short-notice staffing requirements.

Coordinating Health Care Assistants and care support staff.

Supporting staff with day-to-day operational queries.

Monitoring attendance, sickness and staffing requirements.

Liaising with clients, families, healthcare professionals and care staff.

Maintaining accurate care, staffing and scheduling records.

Supporting the Registered Manager and senior management team.

Ensuring care is delivered in accordance with care plans and company procedures.

Supporting quality, safeguarding and compliance requirements.

Participating in an on-call rota.
About You
We are looking for an organised and experienced Care Coordinator with a background in health and social care.
The ideal candidate will have:

Previous experience as a Care Coordinator, Domiciliary Care Coordinator, Care Scheduler, Care Planner or similar.

Experience coordinating care staff and managing rotas.

Experience within domiciliary care, home care or community care.

Excellent administration and organisational skills.

Strong IT and record-keeping skills.

Excellent communication and interpersonal skills.
-
The ability to prioritise workload and remain calm under pressure.

Experience responding to short-notice staffing and scheduling issues.
