Clinical Pharmacist – Hybrid Role (Up to £60,000 FTE)
We are seeking an experienced and motivated Clinical Pharmacist to join a progressive and supportive primary care team within general practice. Role Overview
The successful candidate will work as part of a multidisciplinary team, contributing to high-quality patient care across a range of clinical and medication-related services. The role offers a balance of on-site clinical work and remote flexibility, supporting both professional autonomy and a strong work-life balance.
Key Responsibilities
- Conduct structured medication reviews and chronic disease medication optimisation
- Support long-term condition management in collaboration with GPs and the wider MDT
- Review prescriptions, manage repeat prescribing processes, and ensure medicines safety
- Provide clinical advice on complex medication queries and polypharmacy cases
- Participate in patient-facing consultations both in practice and remotely
- Support quality improvement initiatives and medicines management projects
- Contribute to clinical governance, audits, and service development
- Work collaboratively with GPs, nurses, and allied health professionals

Salary
Up to £60,000 FTE, dependent on experience
What We Are Looking For
- Qualified Clinical Pharmacist with experience in primary care or NHS settings
- Independent Prescriber status (or willingness to work towards)
- Strong communication and clinical decision-making skills
- Ability to work autonomously and as part of a team
