Job Description:
The successful candidate will play a vital role in providing exceptional care to patients, working both autonomously and collaboratively within a multidisciplinary team. The Nurse Practitioner will be responsible for clinical assessments, diagnosis, and treatment of both acute and chronic conditions, ensuring patients receive comprehensive and effective care.

Key Responsibilities:
• Conduct clinical assessments, diagnose health conditions, and develop treatment plans.
• Manage chronic diseases and provide preventative care.
• Prescribe medications and treatments within the scope of practice.
• Perform health screenings and physical exams.
• Collaborate with GPs, nurses, and other healthcare professionals to ensure coordinated care.
• Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
• Provide patient education and support, promoting healthy lifestyle choices.

Qualifications and Experience:
• Registered Nurse Practitioner with a valid NMC PIN.
• Postgraduate qualification in advanced clinical practice or equivalent.
• Experience in primary care, urgent care, or chronic disease management.
• Strong clinical assessment and decision-making abilities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
