Job Summary:
A well-established and patient-focused medical practice in Chelmsford is seeking an experienced and dedicated Practice Nurse to join their dynamic healthcare team. The successful candidate will play a vital role in delivering high-quality care to a diverse patient population.
Key Responsibilities:
• Provide comprehensive nursing care, including wound care, immunisations, long term condition management and health screenings.
• Conduct patient assessments, develop care plans, and support the delivery of patient-centred care.
• Collaborate closely with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure continuity of care.
• Offer patient education and health promotion advice, empowering individuals to take control of their health and well-being.
• Maintain accurate and up-to-date patient records, ensuring compliance with all relevant guidelines and protocols.
