Job Summary:

A thriving and patient-centred medical practice in Stoke-on-Trent is seeking an experienced and compassionate Practice Nurse to join their dedicated healthcare team. The successful candidate will play an integral role in delivering high-quality care to a diverse and growing patient population.

Key Responsibilities:

Provide a wide range of nursing care services, including wound management, immunizations, and routine health screenings.
Perform patient assessments, develop tailored care plans, and deliver patient-focused care.
Collaborate with GPs, healthcare assistants, and other multidisciplinary team members to ensure seamless continuity of care.
Deliver health promotion and education, empowering patients to take charge of their well-being.
Maintain accurate, detailed patient records while adhering to all relevant clinical guidelines and standards.

Qualifications and Experience:

Registered Nurse (RGN) with an active NMC pin.
Prior experience in a primary care or community healthcare setting is highly preferred.
Competent in administering child and adult immunizations and performing cervical smear tests.
Experience in managing chronic diseases is an advantage.
Strong communication skills with the ability to work collaboratively in a team environment.
Proficient in using electronic patient records and clinical systems.
