Role Summary:
The practice is seeking a skilled and compassionate Practice Nurse to join its dynamic team. The successful candidate will play a key role in delivering a wide range of nursing services, including chronic disease management, immunizations, health promotion, and general practice nursing duties.
Key Responsibilities:
- Deliver high-quality nursing care, including assessment, planning, implementation, and evaluation of patient care.
- Manage long-term conditions such as diabetes, asthma, and hypertension.
- Administer vaccinations and immunizations, including travel vaccinations.
- Conduct cervical screening and other health checks.
- Support the GP team with minor illness clinics and triage.
- Provide advice on health promotion and disease prevention.
- Maintain accurate patient records and ensure all care is documented in line with practice policies.
- Participate in clinical audits and contribute to the development of practice protocols.
- Engage in continuous professional development to ensure up-to-date knowledge and skills.
Qualifications and Experience:
- Registered Nurse with a valid NMC PIN.
- Previous experience working as a Practice Nurse in a GP surgery or similar setting is preferred.
- Experience in chronic disease management, immunizations, and cervical screening.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent organizational skills and attention to detail.
