Job Title: Practice Nurse
Location: Salisbury, Wiltshire
Employment Type: Full-time/Part-time

Job Summary:
A well-established and friendly GP practice in Salisbury is seeking a dedicated and compassionate Practice Nurse to join their dynamic healthcare team. The successful candidate will play a crucial role in delivering high-quality care to a diverse patient population, providing a range of clinical services in a supportive and professional environment.
Key Responsibilities:
- Conducting patient assessments, including chronic disease management, wound care, immunizations, and family planning services.
- Performing routine health checks, such as blood pressure monitoring, ECGs, and health screenings.
- Administering vaccinations and injections, including travel immunizations and childhood vaccines.
- Supporting patients with long-term conditions, providing advice and management for conditions such as diabetes, asthma, and hypertension.
- Assisting in the development and implementation of individualized care plans.
- Offering lifestyle advice to patients, including smoking cessation, weight management, and healthy living.
- Collaborating with GPs and other healthcare professionals to ensure integrated and coordinated patient care.
- Maintaining accurate and up-to-date patient records in line with practice policies and procedures.
- Participating in clinical audits and contributing to the continuous improvement of services.
Qualifications and Experience:
- Registered Nurse (RGN) with current NMC registration.
- Previous experience working in a primary care or general practice setting is desirable.
- Proficient in chronic disease management, wound care, and immunizations.
- Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
- A commitment to delivering high standards of patient care and promoting health and well-being.
- Ability to work independently as well as part of a multidisciplinary team.
- Familiarity with electronic patient records and IT systems used in healthcare.
